Classification
Synthetic triple GIP/GLP-1/glucagon receptor agonist
Structure and chemistry
Retatrutide (LY3437943) is a single peptide engineered to activate the GIP, GLP-1, and glucagon receptors. It is designed as a unimolecular triple agonist rather than a physical mixture of three separate peptides. The exact chemical form should be matched to the analytical reference standard.
What the published research shows
Retatrutide has progressed from preclinical triagonist research through first-in-human and Phase 1b studies, a major Phase 2 obesity program, and multiple Phase 3 programs. Current research includes obesity, type 2 diabetes, knee osteoarthritis, obstructive sleep apnea, cardiovascular and kidney outcomes, and metabolic-associated steatotic liver disease.
Mechanism of action
A single molecule with agonist activity at GIP, GLP-1 and glucagon receptors. The scientific rationale is to combine incretin effects with glucagon-receptor activity in one balanced molecular framework.
Development history
The scientific foundation for unimolecular GIP/GLP-1/glucagon triagonists predates retatrutide. Lilly subsequently developed LY3437943 and published discovery, receptor characterization and clinical proof-of-concept work before moving into Phase 2 and the large TRIUMPH/TRANSCEND Phase 3 programs.
Current regulatory status
Retatrutide remains investigational and is not FDA-approved. As of September 2026, Lilly describes its clinical development as Phase 3.
Analytical testing
HPLC/RP-HPLC and LC-MS provide complementary information for chromatographic purity and molecular identity. A COA should identify the exact molecular target and distinguish purity from content and identity.
