Classification
Synthetic pentapeptide growth hormone secretagogue
Structure and chemistry
Ipamorelin is the pentapeptide Aib-His-D-2-Nal-D-Phe-Lys-NH2, where Aib is alpha-aminoisobutyric acid and D-2-Nal is D-2-naphthylalanine. It contains unnatural amino acids and D-configuration residues, which is why FDA has specifically noted that its characterization is analytically complex. It was derived from GHRP-1 by removing the central Ala-Trp dipeptide.
What the published research shows
The evidence base is almost entirely preclinical. The foundational 1998 Novo Nordisk paper covers in vitro work in primary rat pituitary cells and in vivo work in conscious and anaesthetised swine. There is no published human efficacy trial establishing growth-hormone-related clinical benefit, and the compound was never carried to regulatory approval.
Mechanism of action
A selective agonist at the growth hormone secretagogue receptor subtype 1a (GHS-R1a), the receptor later identified as the ghrelin receptor. It stimulates pituitary GH release through a GHRP-like pathway distinct from the GHRH receptor.
Development history
Developed at Novo Nordisk (Denmark) under the designation NNC 26-0161 as part of a program seeking growth hormone secretagogues with a cleaner endocrine profile than GHRP-6, GHRP-2 and hexarelin.
Current regulatory status
Ipamorelin is not approved by the FDA or any other regulator for any indication, and Novo Nordisk did not carry it to market. In September 2023 FDA placed ipamorelin acetate in Category 2 of its 503B interim bulk drug substances list; its 503A nomination was subsequently withdrawn.
Analytical testing
Identity should be verified against the intended sequence and stereochemistry by mass spectrometry, with particular attention to the Aib and D-2-Nal residues. HPLC purity is a separate measurement from molecular identity and does not confirm D/L configuration.
