Classification
Fixed blend of a growth hormone-releasing hormone analogue and a selective growth hormone secretagogue
Structure and chemistry
Tesamorelin is a 44-amino-acid stabilised analogue of human growth hormone-releasing hormone carrying a trans-3-hexenoyl group on the N-terminus, which resists degradation by dipeptidyl peptidase-4. Ipamorelin is a pentapeptide, Aib-His-D-2-Nal-D-Phe-Lys-NH2, acting at a different receptor. The blend supplies both in one vial.
What the published research shows
Tesamorelin has the stronger evidence base of the two, with randomised trials in HIV-associated lipodystrophy that supported its approval. Ipamorelin was developed at Novo Nordisk and studied for postoperative ileus without reaching approval; it is noted in preclinical work for releasing growth hormone with less effect on cortisol and prolactin than earlier secretagogues. The fixed combination has not been studied as such in controlled trials.
Mechanism of action
Tesamorelin acts at the GHRH receptor on pituitary somatotrophs. Ipamorelin acts at the growth hormone secretagogue receptor, the ghrelin receptor. The rationale for combining them is that the two pathways are complementary, though that rationale has not been tested clinically as a fixed blend.
Development history
Tesamorelin is FDA-approved as Egrifta for a specific indication in HIV patients. Ipamorelin has no approval. The blend is not an approved product.
Current regulatory status
Supplied for laboratory research use only. Not an approved combination product.
Analytical testing
Confirmation by HPLC for the presence and ratio of both peptides is the relevant analysis, since a single purity figure for a blend does not establish how much of each component is present.
