Classification
Synthetic hexapeptide GH secretagogue; the only GHRP with any national regulatory approval
Structure and chemistry
GHRP-2, also known as pralmorelin, is the hexapeptide D-Ala-D-2-Nal-Ala-Trp-D-Phe-Lys-NH2, where D-2-Nal is D-2-naphthylalanine. It is a second-generation analogue engineered from the GHRP-6 scaffold for greater GH-releasing potency per unit dose. The unnatural naphthylalanine residue is specifically cited by FDA as adding complexity to peptide characterization.
What the published research shows
GHRP-2 is the most clinically validated compound in the GHRP family, though the volume is still modest — roughly 15 to 20 human studies. Its validated use is diagnostic rather than therapeutic: it produces a robust, reproducible GH response in healthy subjects that is markedly blunted in GH-deficient patients, making it a provocative test agent. Head-to-head work by Arvat and colleagues in 1997 compared it directly against hexarelin, GHRH, TRH and hCRH in healthy young adults.
Mechanism of action
A potent GHS-R1a (ghrelin receptor) agonist stimulating GH release from anterior pituitary somatotrophs. More potent than GHRP-6 per microgram with less appetite stimulation, but still less selective than ipamorelin with respect to cortisol and prolactin.
Development history
Developed from the Tulane University series led by Cyril Bowers, in association with Polygen in Germany. Kaken Pharmaceutical acquired worldwide manufacturing and marketing rights and sublicensed US and Canadian rights to Wyeth (formerly American Home Products); Wyeth’s US development for GH deficiency was discontinued, while Kaken retained the Japanese rights.
Current regulatory status
GHRP-2 holds a distinction no other growth hormone secretagogue has: a national regulatory approval. Japan’s regulator approved pralmorelin (marketed by Kaken) in 2004, strictly as a diagnostic agent for assessing growth hormone deficiency. It is not FDA-approved. FDA placed GHRP-2 (injectable and nasal routes) in Category 2 of the 503B interim bulk drug substances list in September 2023.
Analytical testing
Mass spectrometry is required to confirm the D-2-naphthylalanine residue and the D-amino acid configurations. FDA has specifically flagged this unnatural amino acid as a characterization challenge, so a bare HPLC purity percentage is insufficient for identity.
