Classification
Synthetic hexapeptide GH secretagogue with a distinct cardiac binding target
Structure and chemistry
Hexarelin, also called examorelin, is the hexapeptide His-D-2-MeTrp-Ala-Trp-D-Phe-Lys-NH2. It differs from GHRP-6 by a single methyl group: the second residue is 2-methyl-D-tryptophan rather than D-tryptophan, a modification that increases metabolic stability and potency.
What the published research shows
Hexarelin has been studied both as a GH secretagogue and, distinctly within this family, for cardiovascular effects. Research has identified binding to CD36, a scavenger receptor expressed in cardiac tissue, as a target separate from GHS-R1a — which is why hexarelin appears in cardiac research contexts where the other GHRPs do not. Human studies have been conducted, but no therapeutic development program was completed.
Mechanism of action
A potent GHS-R1a agonist, and additionally a ligand at CD36 in cardiac tissue. Comparative human work found hexarelin and GHRP-2 produced GH responses exceeding those of maximal-dose GHRH, with slight stimulatory effects on prolactin and ACTH/cortisol.
Development history
Developed from the Bowers GHRP scaffold by Romano Deghenghi and colleagues, published in Life Sciences in 1994. It emerged from the same structure-activity effort that produced GHRP-1 and GHRP-2 from the original GHRP-6 hexapeptide.
Current regulatory status
Not approved by the FDA or any other regulator for any indication. Unlike GHRP-2 and GHRP-6, hexarelin has not been placed on FDA’s bulk drug substances lists, which reflects nomination history rather than any FDA assessment.
Analytical testing
Identity confirmation requires mass spectrometry able to distinguish the 2-methyl-D-tryptophan residue from ordinary D-tryptophan — the difference between hexarelin and GHRP-6 is a single methyl group, roughly 14 daltons, which HPLC purity alone will not resolve.
